Why do favorites icons keep changing

Very frequently I have a problem with my bookmark icons being changed. I have a screenshot attached and if you will notice the icon in the Bookmarks Bar for Bleeping Computer is not as it should be.

Almost all the icons in every folder look like that. It almost looks like a black and white world globe. Then after I went to each site and clicked the star to bookmark it in order to change the icon, a lot of them had been changed to the symbol for my bank.

This has been going on for months. I finally started exporting them in HTML to an external drive. But it is a real hassle to delete all of them, then re-import then every couple of weeks. Is this a virus, or some screw up in the OS?

Because,I tried to right-click on the favorite bar and I choosed the first option " open all 3 " but it didn't load all bookmarks. I mean that many icons are not yet correct. They are not correct because the action you performed in the linked post removes Chrome's memory of the icons, so you have to revisit the pages in order for Chrome to GET the new icons.
